Patient Care Coordinator

About the role

SDKare LLC is seeking a dedicated Patient Care Coordinator to lead its Medical Administrative Assistant team. The role ensures smooth scheduling, prior authorizations, and overall administrative operations while delivering high‑quality, confidential patient service.

Key responsibilities

• Schedule new and follow‑up patient appointments accurately, matching patients with appropriate providers.
• Verify appointment details, send confirmations, reminders, and next‑step communications.
• Review and complete patient registrations, ensuring all demographic and identification data are accurate.
• Collect and verify required questionnaires, consent forms, and intake documents before appointments.
• Maintain and update patient records in the EHR system promptly.
• Respond to patient inquiries via phone, email, SMS, and chat, and follow up on missing documentation.
• Escalate clinical questions and urgent concerns to the appropriate provider or department.
• Coordinate communication among patients, providers, pharmacies, and internal teams.
• Assist providers with medication follow‑up requests, chart notes, and prior authorizations.
• Ensure compliance with HIPAA, company confidentiality policies, SOPs, and quality standards.

Required profile

• Bachelor’s degree or equivalent.
• Minimum 1 year of experience in customer service, healthcare coordination, or telehealth.
• Strong verbal and written English communication skills.
• Excellent organizational and multitasking abilities.
• Ability to work independently and manage competing priorities.

Required skills

• Proficiency with EHR/EMR systems (e.g., Charm EHR).
• Microsoft Office applications.
