Population Health Support Specialist

About the role

The Population Health Support Specialist will work from EDGE’s Lahore office to ensure that patients, providers, and clinics are fully prepared for every appointment. This non‑clinical position supports healthcare providers by managing chart readiness, identifying care gaps, and coordinating follow‑up activities to improve patient outcomes and operational efficiency.

Key responsibilities

• Review upcoming appointments and verify that charts are complete and visit‑ready.
• Identify preventive‑care opportunities and chronic‑condition care gaps before visits.
• Check labs, referrals, imaging and follow‑up requirements, flagging missing information for the appropriate teams.
• Contact patients to schedule follow‑up appointments, screenings, laboratory work, imaging and referrals.
• Document all outreach and patient interactions accurately in the Electronic Health Record (EHR) system.
• Track outstanding patient needs, support care coordination and assist with referral management.
• Maintain accurate tracking tools for care gaps, outreach and quality initiatives.
• Enter and maintain data in NextGen and other clinical systems while ensuring HIPAA compliance.

Required profile

• Must work on‑site in Lahore, Monday‑Friday, 8:00 AM‑5:00 PM PST.
• At least one year of experience in a US healthcare environment.
• Strong organizational, documentation and time‑management abilities.
• Ability to work independently and meet productivity goals.

Required skills

• Experience with Electronic Health Records (EHR).
• Familiarity with the NextGen clinical system.
