Senior Project Manager – Chronic Care Management & RPM

About the role

We are looking for a proactive Senior Project Manager with deep expertise in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) solutions. The role involves leading cross‑functional teams to deliver healthcare software initiatives that meet clinical, regulatory, and business requirements.

Key responsibilities

• Lead end‑to‑end project lifecycles for CCM and RPM software, from requirements gathering to production release.
• Act as the liaison between clinical stakeholders, business units, and technical teams to ensure accurate translation of requirements.
• Define project scope, objectives, milestones, and deliverables aligned with compliance goals.
• Develop and maintain detailed project plans, timelines, resource allocations, risk logs, and status reports.
• Facilitate Agile ceremonies such as daily stand‑ups, sprint planning, backlog grooming, reviews, and retrospectives.
• Document business and functional requirements, user stories, workflows, and acceptance criteria.
• Ensure compliance with CMS guidelines, HIPAA, and other healthcare regulations.
• Oversee integration with EHR/EMR systems, medical devices, and third‑party platforms.
• Monitor risks, dependencies, and change requests, driving timely resolutions.
• Collaborate with QA teams for testing, validation, and user acceptance.
• Provide regular updates to clients, providers, care coordinators, and internal stakeholders.
• Support post‑deployment activities, including training, documentation, and process optimization.

Required profile

• Bachelor’s or Master’s degree in Computer Science or a related field.
• 7+ years of experience as a Project Manager or Business Analyst in healthcare technology.
• Proven experience delivering CCM and/or RPM programs.
• Strong knowledge of Agile/Scrum methodologies and the Software Development Lifecycle.
• Excellent stakeholder management, communication, and leadership abilities.
• Ability to manage multiple concurrent projects in a fast‑paced environment.
