Medical Scribe – Clinical Documentation Specialist

About the role

We are seeking a focused Medical Scribe to support physicians by documenting patient encounters in real time. The role involves accurate entry of clinical information into electronic health record (EHR) systems and ensuring all documentation meets clinic standards.

Key responsibilities

• Document History of Present Illness, physical exam findings, assessments, and treatment plans during clinic visits.
• Enter and update patient data in Practice Fusion and Office Ally EHR platforms under physician supervision.
• Prepare patient charts before appointments, ensuring all relevant records are available.
• Maintain and update follow‑up notes, referrals, and other clinical documentation.
• Guarantee timely, complete, and accurate clinical documentation in line with clinic policies.
• Uphold patient confidentiality and comply with HIPAA regulations.
• Collaborate with front‑desk staff, physicians, and billing teams to resolve documentation queries.

Required profile

• Medical or healthcare‑related degree.
• Minimum 2 years experience as a Medical Scribe.
• Hands‑on experience with Practice Fusion and Office Ally EHR systems.
• Excellent typing speed and accuracy.
• Strong listening, comprehension, and English language skills.
