Case Worker – Community Health Integration

About the role

We are seeking compassionate, mission‑driven Case Workers to join our Community Health Integration (CHI) team. In this role you will identify social determinants of health (SDOH) barriers that limit patients’ access to care, connect them with appropriate community resources, and provide ongoing support and follow‑up.

Key responsibilities

• Conduct structured SDOH needs assessments for patients.
• Link patients to housing, food, transportation, utility and other community resources.
• Provide continuous case management and follow‑up to ensure resource utilization.
• Maintain accurate, compliant documentation of all interactions.
• Coordinate with clinical providers and multidisciplinary care teams.
• Communicate professionally and empathetically with patients.

Required profile

• US healthcare experience is mandatory.
• Experience as a CHI Worker, Case Worker, or Social Worker in a US healthcare setting is preferred.
• Strong knowledge of SDOH and community resource navigation.
• Excellent verbal and written English communication skills.
• Strong documentation, organization, and follow‑up abilities.
• Empathetic, patient‑focused approach.

Required skills

• HCPCS coding (e.g., G0019, G0022).
• SDOH assessment tools.
