Population Health Support Specialist

About the role

We are seeking a Population Health Support Specialist to ensure that patients, providers, and clinics are fully prepared for every appointment. This non‑clinical, in‑office role supports healthcare providers by preparing charts, identifying care gaps, and coordinating follow‑up activities to improve patient outcomes and operational efficiency.

Key responsibilities

• Review upcoming patient appointments and verify that charts are complete and visit‑ready.
• Identify preventive care opportunities and chronic condition care gaps.
• Check labs, referrals, imaging, and follow‑up requirements before visits.
• Flag missing documentation or clinical information for the appropriate teams.
• Contact patients to schedule follow‑up appointments, screenings, lab work, imaging, and referrals.
• Document outreach activities and patient interactions accurately in the Electronic Health Record (EHR).
• Support care coordination by tracking outstanding patient needs and ensuring appropriate follow‑up.
• Assist with referral management and provider‑requested follow‑up activities.
• Maintain accurate tracking tools related to care gaps, outreach, and quality initiatives.
• Enter and maintain documentation within NextGen and other clinical systems.
• Support quality improvement initiatives and audit readiness while maintaining HIPAA compliance.

Required profile

• Willingness to work onsite in Islamabad, Monday‑Friday, 8:00 AM‑5:00 PM PST.
• At least 1 year experience in a US healthcare environment.
• Strong organizational, documentation, and time‑management abilities.
• Ability to work independently and meet productivity goals.

Required skills

• Experience with Electronic Health Records (EHR).
• Familiarity with NextGen clinical system.
