Senior AI Engineer - emrgen.ai

About the role

iCareManager is a HIPAA-compliant, SOC 2 Type II certified EHR and management platform for IDD, assisted living, ICF and human services providers, and it builds a family of AI products. emrgen.ai is its clinical documentation and EMR product, built with an outside engineering vendor and now being brought fully in-house. The company needs one senior engineer in Lahore to take over the code, infrastructure, models and prompts, finish the handover with the vendor, then decide the product's next steps. The role reports to the Chief Product Officer and is hybrid.

Key responsibilities

• Take over the emrgen.ai codebase, cloud environment and AI components from the vendor, with full documentation, clean access and no single point of failure.
• Audit current architecture, security, data handling and model behavior, and deliver a written plan for what to keep, fix or rebuild.
• Run the AI documentation features end to end, from clinician input to the structured note a clinician reviews and signs.
• Evaluate AI quality with real test sets, tracking accuracy, invented content and how often clinicians edit generated notes.
• Build guardrails so the AI does not add clinical facts not in the source, and requires clinician approval for every note.
• Protect patient data with access controls, audit logging and rules on what may reach outside model providers.
• Connect emrgen.ai to the iCareManager platform and to state Medicaid billing requirements.
• Set up releases, monitoring and on-call coverage.
• Mentor engineers on the Pakistan team and help hire new members.

Required profile

• 8+ years as a professional software engineer, including time as a technical lead.
• 3+ years shipping AI features into production, not demos.
• Experience building or heavily working on an EHR or EMR system for U.S. healthcare customers while based in Pakistan, with the system named.
• Experience inheriting a system and making it maintainable.
• Written English strong enough for customer-facing documentation and comfort with U.S. hours overlap.

Required skills

• Large language models: prompt design, retrieval, evaluation, and controlling cost and speed.
• Speech-to-text experience is a strong plus.
• Working knowledge of HIPAA, audit trails and protected health information in AI pipelines.
• Nice to have: experience in IDD, behavioral health or Medicaid billing; HL7 or FHIR; SOC 2 audit support; machine learning operations such as model versioning and monitoring.
