Chronic Care Management Specialist

About the role

This full‑time, night‑shift position is based on‑site in Johat Town, Lahore. The Chronic Care Management Specialist will provide high‑quality support to patients with chronic conditions, ensuring continuous coordination between patients, healthcare providers, and the care team.

Key responsibilities

• Engage patients regularly to monitor health status and adherence to care plans.
• Track and update individual chronic care plans, ensuring accuracy and timeliness.
• Facilitate communication between patients, physicians, and other healthcare professionals.
• Provide excellent customer service and resolve patient inquiries promptly.
• Support the coordination of chronic care initiatives and assist with medical billing processes when needed.

Required profile

• Strong interpersonal and customer‑service abilities.
• Excellent English communication skills for interaction with patients and providers.
• Basic understanding of medicine and critical‑care management practices.
• Ability to work collaboratively on patient‑centered care initiatives.
• Bachelor’s degree in Pharm‑D or related field.
• At least 1 year of experience in chronic care management or a related healthcare role.
