Medical Scribe Specialist

About the role

Sequel Technologies, a software company providing healthcare IT and medical billing services to the US market, is hiring a Medical Scribe Specialist based in Bahria Town Phase 7, Rawalpindi (residents of Bahria, DHA or nearby areas preferred). The role involves documenting patient encounters in real time in the electronic health record (EHR) and supporting accurate coding and billing.

Key responsibilities

• Document patient encounters accurately and in real time in the EHR, including HPI, ROS, physical examination findings, assessment and treatment plans.
• Navigate charts to find patient records, lab results, imaging reports, medication lists and prior notes, and enter and update clinical notes following the provider's instructions.
• Capture documentation elements needed for charge capture and coding: chief complaint, history, exam, medical decision making and time spent.
• Support order entry, referrals and results review only as directed by the provider, and coordinate with coders and billing staff on documentation queries affecting claims.
• Maintain patient privacy and comply with HIPAA requirements and data-handling policies, including in remote settings.
• Proofread notes for spelling, grammar and logic, report EHR errors and system issues to the supervisor, and finish documentation within agreed turnaround times.

Required profile

• MBBS or another relevant medical degree; other relevant medical qualifications considered.
• 1 to 2 years of experience as a medical scribe, clinical documentation specialist or clinical assistant.
• Good understanding of medical terminology, anatomy, common diagnoses, medications and clinical documentation.
• Strong written and verbal English, including listening comprehension.
• Preferred: real-time or remote scribing experience, experience with US healthcare providers and practice workflows, and specialty-specific documentation experience.

• EHR proficiency: entering and updating clinical notes in electronic health record systems.
• Attention to detail and confidentiality awareness.
