Medication Adherence & Quality Performance Specialist

About the role

The Medication Adherence & Quality Performance Specialist ensures high‑quality care for assigned patients, supports regulatory reporting, and helps the Population Health Team meet value‑based care goals.

Key responsibilities

• Conduct outreach to medical practices, providers, and patients to promote health screenings, chronic‑condition management, and well‑care visits.
• Engage patients by phone, provide education, navigate benefits, and arrange needed care to improve compliance.
• Identify workflow gaps in provider practices, suggest best practices, and perform virtual visits as needed.
• Collect, enter, and abstract medical record data with high accuracy, supporting HEDIS and payer quality measures.
• Build and maintain professional relationships with health‑plan representatives, provider practices, and internal departments.
• Generate executive reports and presentations on key performance indicators.

Required profile

• Pharm.D degree from a reputable university.
• At least 1 year of experience with Microsoft Word and Excel for reporting.
• Basic understanding of the healthcare industry and HIPAA compliance.
• Ability to work independently, meet tight timelines, and pay close attention to detail.
