Pharmacist – Clinical Pharmacy Role

About the role

Our client Work Together is seeking a qualified Pharmacist to provide comprehensive pharmaceutical care, manage medication orders, and collaborate with healthcare professionals to ensure optimal patient outcomes.

Key responsibilities

• Review and execute physician prescriptions, ensuring appropriateness and legality.
• Provide medication therapy management, safety monitoring, and patient counseling.
• Ensure compliance with state and federal regulations.
• Collaborate with healthcare teams to optimize patient outcomes.
• Monitor medication adherence and conduct drug utilization reviews.
• Manage prescription and non‑prescription orders, including refills.
• Utilize digital healthcare systems for efficient pharmacy operations.
• Document and communicate consultation outcomes to healthcare teams.
• Stay updated with pharmaceutical research, advancements, and guidelines.

Required profile

• Valid pharmacist license in the practicing state.
• Bachelor’s degree in Pharmacy or Pharm.D.
• 3–5 years of experience in a pharmacy setting.
• Excellent communication, decision‑making and problem‑solving abilities.
• Certification in Basic Life Support (BLS) is desirable.
• Commitment to ongoing education and professional development.

Required skills

• Proficiency with pharmaceutical databases and software.
• Experience using digital pharmacy tools for remote work.
• Familiarity with electronic health record (EHR) systems.
