Medical Biller - BHRF Specialist

About the role

Connexus Cure is looking for an experienced Medical Biller specializing in Behavioral Health Residential Facilities (BHRF) to join its team in Gulberg, Lahore. The role covers U.S. healthcare billing for residential behavioral health treatment facilities, including insurance claims, denial management and accounts receivable follow-up. Candidates with direct experience handling BHRF claims for U.S. payers, including Medicaid and commercial plans, are strongly preferred.

Key responsibilities

• Manage BHRF billing by reviewing and submitting claims according to payer requirements
• Prepare and submit accurate electronic and paper claims with correct procedure codes, revenue codes, modifiers and billing details
• Verify patient eligibility, benefits and coverage limitations, including residential behavioral health treatment benefits
• Track prior authorizations, continued-stay approvals and authorization limits to reduce denials
• Investigate and resolve denied claims, including authorization, eligibility and documentation issues
• Follow up with insurers on unpaid, underpaid and outstanding claims and post payments, adjustments and write-offs
• Work with payer portals and clearinghouses to check claim status, review remittance advice and manage correspondence
• Understand BHRF billing codes, including HCPCS codes such as H0018, depending on payer, service and contract
• Coordinate with facility and clinical teams for documentation, maintain billing records and prepare AR aging, claim status and denial reports
• Follow U.S. billing regulations, payer policies, HIPAA and internal procedures

Required profile

• Proven U.S. medical billing experience, specifically with BHRF or residential behavioral health facilities
• Strong understanding of Medicaid and commercial insurance workflows
• Experience with claims submission, payment posting, denial resolution and AR follow-up
• Familiarity with behavioral health billing codes, authorization requirements and payer guidelines
• Good written and spoken English for communicating with U.S. insurers and clients
• Preferred: experience with U.S. BHRF facilities and their Medicaid managed care or commercial plans

• Medical billing software, EHR/EMR systems, clearinghouses and payer portals
• Analytical, communication and problem-solving skills; ability to work independently and meet deadlines
