About the role
We are looking for a dedicated CCM Patient Support Representative to join our team on‑site in Lahore on a night‑shift schedule. The role focuses on assisting patients enrolled in Chronic Care Management programs, ensuring they receive timely support and guidance.
Key responsibilities
- Handle patient inquiries and provide support related to chronic care management.
- Follow established CCM workflows to coordinate care activities.
- Communicate with patients in clear, professional English, both verbally and in writing.
- Utilize EMR/EHR systems to document interactions and update patient records.
- Maintain a professional attitude while working night‑shift hours.
Required profile
- Proven experience as a Patient Support Representative.
- Strong knowledge of CCM workflows.
- Excellent English communication skills (verbal & written).
- Professional attitude and ability to work night shifts.
- Experience with EMR/EHR systems is preferred.
