Patient Care Coordinator (Onsite, Evening Shift)

About the role

Personic Health, a US-based in-home wound care center, is looking for a highly organized and tech-savvy Patient Care Coordinator to support its growing operations. The role keeps patients moving efficiently through their care plan while coordinating across internal clinical teams and external partners. It is a full-time, onsite position in Karachi, Pakistan, working evening and night shifts from 7:00 PM to 3:00 AM PKT.

Key responsibilities

• Serve as the central point of coordination for patient care across clinicians, internal support teams and external vendors, ensuring a smooth, timely patient experience from the first visit through ongoing care.
• Manage high-volume follow-ups so there are no gaps in care or communication.
• Own and manage patient scheduling and routing end to end, optimizing schedules for timely visits and efficient provider routes, and handling real-time schedule changes and urgent updates.
• Document all patient interactions accurately and in real time, working across multiple systems each day: EMR, CRM, and scheduling and mapping tools.
• Collaborate with clinicians (NPs, wound nurses, wound techs), data/intake teams and scribes, and coordinate with home health agencies, DME providers and mobile labs and ancillary vendors, following up on orders, referrals and patient status updates.

Required profile

• Bachelor's degree in healthcare, Health Information Management or a related field.
• Proven experience in patient care coordination, scheduling or healthcare operations.
• Strong communication and interpersonal skills, with a high level of organization, attention to detail and accountability.
• Comfortable working in a fast-paced, high-volume environment.

Required skills

• Tech-savvy, able to work with multiple systems at once.
• Experience using EMR and CRM systems.
